- Beginning March 24, 2018, students in approved periods of Post-Completion or STEM OPT can log-in to the OPT Portal and submit updates directly to SEVIS.
- If you have not received access to your OPT Portal and have an approved period of OPT, please complete the OPT Portal Access Form
- This OPT Employment Report Form will not be available after July 31, 2018.
- Beginning August 1, all updates will be made directly to SEVIS at the OPT Portal.

This form is for UNT students who are currently on Optional Practical Training (OPT) or STEM OPT.
You are required by U.S. immigration regulations to report any change to the information below to International Student and Scholar Services within 10 days, including any end to employment.
Additionally, students on STEM OPT are required to send a report validating the information below to our office every 6 months, beginning the date the STEM OPT starts.
Note that by submitting this form, you are not submitting a request for a new I-20 with updated employer information.
If you require a new I-20 issued with any updated employer information, you must fill out and submit this form, checking the "Other" box as your reason and detailing what employment information needs updating.
