UNT Approved Agent - Student Verification Form

Please complete this form to confirm the students who have been sent to University of North Texas by your agency. This form must be completed AFTER submitting a student application. Please complete this form for each student you have referred to University of North Texas.

This form is to be used for UNT approved agents student verification only.

Submit the student’s UNT and/or IELI application.
Fill out this UNT Approved Agency Student Verification form with your agency name (as listed on the agreement), contact person’s name, e-mail address, and student information.
On the “Student Application” line, select either UNT or IELI Application. (Note: If the student is applying to both UNT and IELI, select IELI)
After you click “Next Page,” you will fill out the student’s information.
Click “Next Page” to complete the form.
The final page is a confirmation that you have completed the verification form and submitted it to us. Thank you!
